Case Manager

Central East Community Care Access Centre (CECCAC)

The CECCAC is mandated to provide community-based health and related social services, supplies and equipment; manage the placement of persons into long-term care, and; provide information to the public about community based services, long-term care facilities and related health and social services with the central East Local Health Integration Network, including Haliburton County.  The CECCAC has one of the largest populations served by a CCAC, responding to the needs of approximately 1,459,800 residents, making up 11.8% of Ontario’s population.

To fulfill the above mandate, CECCAC employs Case Managers.  The organization believes that everyone is able to provide leadership and their challenge is to uncover and enable the natural capacity that every employee has, aiming to achieve maximum individual and collective performance towards the realization of our common vision and mission.

The Case Manager position requires a nurse, therapist or a social worker with a relevant related degree/diploma.  Individuals must be currently licensed or registered according to the requirements of the profession in the province of Ontario and have two to three years of community health or related clinical experience.  Fluency in languages other than English would be an asset.
