Haliburton Highlands Health Services (HHHS)
Within the Mental Health Services at Haliburton Highlands Health Services, the Counsellor/Case Manager provides psychosocial assessments, treatment, and case management/therapy services to individuals and families accepted as clients. The Mental Health Counsellor/Case Manager also provides consultative services to families, community professionals and the broader community on mental illness. The Counsellor/Case Manager at HHHS has at minimum a Bachelor’s degree in Social Work, Psychology or related discipline, preferrably a Master’s degree, and demonstrated related experience and knowledge of mental illness and working with individuals who have a severe mental illness/mental health problems/serious dysfunction.
Haliburton Highlands Family Health Team (HHFHT)
The FHT Social Worker is a specialist in assessment, treatment and evaluation of individual, interpersonal and societal issues and responsible for the planning, coordination and delivery of social services to indiviuals, dyads, families, groups and populations. Working in collaborationwith an interdisciplinary team, the Social Worker will use systems and strengths perspectives to assist clients in obtaining optimal health. The FHT Social Worker has a Baccalaureate of Social Work, has a current registration with the Ontario College of Social Workers and Social Serve Workers and demonstrated 3 years of experience in a community or primary care setting.
